Medical practice letterhead, free to download
Referral letters and patient correspondence need the practice name, the address, and a phone number that a person can find in one glance. The rest of the page should stay quiet.
Name the practice, not a campaign
Put the practice name on the first line and the specialty, if you want it, on the second: primary care, dermatology, pediatric dentistry. Patients and referring physicians are looking for who sent the letter, not a slogan.
Use the street address, the main phone number, a general inbox, and the website. A fax number still matters in some referral workflows. The phone field can read “Phone (512) 555-0164 · Fax (512) 555-0165” if you need both on one line.
Leave clinical claims off the sheet
Do not print outcomes, insurance boasts, or patient instructions on the letterhead. Those belong in a letter or a handout, where they can be updated. National provider numbers and license numbers are optional. Many practices leave them off the sheet and add them in the letter when a payer asks.
The letterhead is not the place for a patient’s information. It is blank stationery. The letter you write on it is the record.
Type and color that stay readable
The Practice template puts a thin color bar at the top and the phone, email, and website in a solid footer, so a busy desk can still find the number. Sage, navy, and black all stay legible. Avoid pale gray type. A referring office may photocopy the letter.
